When to See a Periodontist

Many patients with early gum disease have no pain at all. If any of these sound familiar, it's worth a consultation — early treatment is significantly more effective than treatment after bone loss has occurred.

Gums that bleed when brushing or flossing
Persistent bad breath or a bad taste
Teeth that feel loose or have shifted
Teeth that look longer than before
Gums that are red, swollen, or tender
An uneven or excessive "gummy" smile
Pus between the teeth and gums
Existing implants that feel uncomfortable
Your Gum Health Journey

What Happens When Your Gums Need Treatment?

Bleeding gums are a sign your gums need attention — but the right next step depends on what we find at examination, not on the symptom alone. Here's the general path most patients follow, from first visit to long-term maintenance.

  1. Bleeding or Swollen Gums

Bleeding while brushing, persistent bad breath, swelling, or tenderness are usually the first sign that gums need attention.

  2. Examination & Diagnosis

We check pocket depths around each tooth, bleeding on gentle probing, and plaque levels — with dental X-rays where bone levels need to be assessed.

    What we check

A full periodontal examination measures pocket depth at several points around every tooth, checks for bleeding on probing, tooth mobility, and plaque control, and reviews your medical history for factors — such as smoking or diabetes — that affect healing. X-rays are taken when bone levels need a closer look.

  3. Initial Gum Treatment

Professional cleaning and, where indicated, deep cleaning below the gum line — scaling and root planing — paired with personalised home-care guidance.

    Why this step matters

Thorough mechanical cleaning — removing plaque and calculus and smoothing root surfaces — is the evidence-based foundation of gum treatment for almost every patient. It isn't a warm-up before the "real" treatment; for most people, it's the step that does the most work. Laser adjuncts, when used, are added to this foundation rather than replacing it.

  4. Decision Point

Are the Gums Healing and Stable?

A few weeks after initial treatment, we reassess. From here, one of two things is usually true.

    • Yes — Maintenance

Gums have calmed down, pockets are shallow, and bleeding has resolved. The focus now shifts to home care and periodic professional reviews to keep it that way.

    • Not Yet — Reassessment

Some areas are still inflamed, bleeding, or haven't fully responded. We look more closely at exactly which sites need further attention.

  5. Decision Point

What Do the Remaining Findings Show?

If a few areas still need attention, the next step depends on how deep and how persistent they are — not on a single fixed rule.

    • Further Non-Surgical Care

Additional cleaning below the gum line, sometimes with laser-assisted disinfection as an adjunct — used selectively, not as an automatic upgrade for every patient.

      Possible treatment options

This may include repeat subgingival instrumentation of specific sites and, where clinically appropriate, laser-assisted disinfection to reduce bacteria in pockets that are hard to reach mechanically. As a general guide, sites that stay shallow and stop bleeding are usually considered stable, while sites that remain deep — often in the region of 5–6mm — or continue to bleed are more likely to need this extra step. Pocket depth is only one of several factors we weigh.

    • Deeper or Persistent Areas

For a small number of sites that don't respond to non-surgical care, localized gum surgery may be considered — with bone grafting or regenerative treatment where the defect supports it.

      Possible treatment options

This may include localized flap surgery to access and clean deep-seated deposits, guided tissue regeneration using biocompatible membranes, or bone grafting to rebuild lost support. Whether any of these apply — and which one — depends on the depth and shape of the defect, the tooth's restorability, and your overall health, assessed individually at consultation.

  6. Healing Review & Long-Term Maintenance

Whichever path applies, gum health is monitored over time. Consistent professional maintenance and daily home care meaningfully reduce the risk of the disease returning.

This is a general educational guide, not a diagnosis or a fixed treatment plan. Your recommended approach depends on a complete periodontal examination — pocket measurements, bleeding, bone levels, tooth anatomy, your overall health, and how your gums respond to initial treatment.

Gum Care That Goes
Beyond the Surface

Bleeding gums are not normal — they are a signal. At Excellence Dental, our in-house specialist periodontists treat the cause, not just the symptom. Deep cleaning performed by a specialist is a fundamentally different experience from a routine cleaning at a general dental practice.

We have two in-house specialist periodontists — Dr. Pragya Pathak MDS and Dr. Ruthali Kamat MDS — both with postgraduate specialist degrees in periodontics. Patients consistently describe their experience as gentle yet thorough, often remarking that it is unlike any cleaning they have had before. This is the difference a specialist makes.

Gum health is also the foundation of implant success. Every implant patient at Excellence Dental is assessed and treated by our periodontists before and after implant placement.

Periodontal Treatments at Excellence Dental

🦷

Deep Cleaning (Scaling & Root Planing)

The gold-standard non-surgical treatment for gum disease. Removes bacterial deposits from deep below the gum line and smooths root surfaces to help gums heal and reattach. Performed under local anaesthesia for complete comfort.

  • Ultrasonic scalers + fine hand instruments
  • Local anaesthesia — comfortable, not painful
  • Significantly more thorough than routine cleaning

LASER-Assisted Gum Treatment

A dental laser is used as an adjunct to conventional deep cleaning in selected cases. The laser reduces bacteria in deep gum pockets and promotes faster healing of gum tissue. Used where the evidence supports a genuine benefit.

  • Adjunct to thorough conventional protocol
  • Reduces bacteria in deep pockets
  • Promotes faster gum healing
🩺

Gum Surgery (Periodontal Surgery)

For advanced gum disease where non-surgical treatment is insufficient, surgical procedures such as flap surgery allow direct access to deep pockets and bone defects for thorough cleaning and bone regeneration.

  • Flap surgery for advanced periodontitis
  • Bone grafting for regeneration
  • Guided tissue regeneration (GTR)
🔄

Periodontal Maintenance

After active gum disease treatment, a structured maintenance programme prevents recurrence. Typically every 3–6 months, these visits monitor pocket depths, remove new deposits, and reinforce home care.

  • Pocket depth monitoring
  • Professional cleaning every 3–6 months
  • Personalised home care guidance

Our Philosophy

If "laser gum treatment" caught your attention because it sounds precise, modern, and painless — you're not wrong to want that. You deserve the honest version of how it actually works, not a sales pitch.

The Laser Isn't the Treatment.
The Cleaning Is.

Nearly every clinic in this city will offer you "laser gum cleaning" as the answer to your problem. It rarely is. Mechanical debridement — the meticulous, unglamorous work of removing calculus and smoothing root surfaces — is what actually gets you better. We'd rather tell you that plainly than sell you a laser as a shortcut.

De-Mystifying "LANAP"

If you've searched "LANAP Bangalore" or "laser gum treatment," you deserve a precise answer, not a marketing term. LANAP® (Laser-Assisted New Attachment Procedure) is a specific, patented, trademarked protocol that requires a particular pulsed Nd:YAG laser. In the industry, "LANAP" is frequently used — incorrectly — as a generic label for any laser-based gum treatment, including procedures that use an entirely different type of laser.

At Excellence Dental, we do not perform the trademarked LANAP protocol, and we won't pretend otherwise to close a sale. We use a 980nm Diode LASER, which current literature supports as a reliable, cost-effective option for sulcular disinfection and soft-tissue management — precisely targeting diseased pocket lining and bacteria while sealing blood vessels for immediate hemostasis and a more comfortable recovery. We'd rather explain that distinction clearly than let a term imply something we don't do.

The American Academy of Periodontology's 2018 best-evidence consensus statement — the specialty's own review of the randomised trial evidence — found that for periodontitis, it is not evident that adjunctive laser therapy improves the clinical outcomes already achieved by thorough mechanical debridement (scaling and root planing) alone. Read the AAP consensus statement →

Why Mechanical Debridement Is the Mainstay

Despite the genuine capabilities of lasers, the evidence consistently places thorough mechanical debridement — not laser therapy — as the non-negotiable foundation of periodontal treatment.

Calculus removal

Lasers kill bacteria and remove soft tissue, but cannot remove hard, calcified calculus from the root surface

Root smoothening

A clean, smooth root surface — achieved only by thorough mechanical instrumentation — is the biological prerequisite for new tissue attachment

Adjunctive role

Research consistently shows lasers are not a standalone therapy; their value is adjunctive, alongside — not instead of — mechanical debridement

Where LASER Adjuncts Genuinely Add Value

None of this means laser technology is theatre. Added on top of a complete conventional protocol, it offers real, specific benefits — which is exactly why we use it selectively, not as a blanket sales pitch.

Adjunct to deep cleaning

Sulcular Disinfection

The 980nm Diode wavelength is highly absorbed by haemoglobin in diseased gum tissue, allowing selective vaporisation of infected pocket lining and a reduction in bacteria within periodontal pockets — used alongside, not instead of, scaling and root planing.

Surgical & microsurgical procedures

Hemostasis & Precision

The same absorption that disinfects also seals blood and lymphatic vessels as the laser works — meaning less bleeding, less post-operative swelling, and more precise soft-tissue work during grafting and plastic procedures.

Post-operative comfort & healing

Photobiomodulation

Low-level laser energy can stimulate fibroblast proliferation and collagen synthesis, supporting cellular repair. Patients often report a gentler, more comfortable recovery when laser is used as part of the protocol.

Why This Matters to Us

We Won't Sell You a Shortcut That Doesn't Exist

Patients are drawn to "laser" treatment because it sounds precise and painless — and to be clear, done properly, it can be both. What it isn't is a replacement for meticulous mechanical debridement, no matter how it's marketed. We do the unglamorous work first: removing hardened calculus and smoothing root surfaces, the part no laser can do. Then we use the laser to do what it's genuinely good at — disinfecting where instruments can't fully reach, sealing tissue for comfort, supporting the healing that follows.

"This isn't about being behind the curve on technology — we use it, deliberately, exactly where the evidence says it helps. It's about refusing to let a fashionable word stand in for the work that actually gets you better."

Microsurgical Periodontics &
Periodontal Plastic Surgery

Periodontal plastic surgery repairs receding gums and reshapes the harmony between teeth and surrounding soft tissue. As a dedicated microscope centre, Excellence Dental performs these procedures under high-power magnification for accuracy conventional methods cannot match.

Gum Grafting for Receding Gums

If your teeth appear longer than before, or cold water gives a sharp zing at the gumline, the tissue protecting your tooth roots has likely worn away. Grafting is performed under the microscope, using sutures thinner than a human hair for zero movement — stability is the single biggest factor in graft success.

  • Connective Tissue Graft (CTG) — the gold standard for root coverage; donor tissue is harvested and transplanted with microscopic precision for a flawless colour match.
  • Free Gingival Graft (FGG) — builds durable, resilient tissue where thin gums are under constant tension, or around implant sites to guard against peri-implantitis.
  • Donor-less options — advanced grafting substitutes are available for patients who prefer to avoid a palatal harvest site.

Gingival Contouring & Crown Lengthening

For an uneven gum line or a "gummy" smile, combining laser technology with microsurgery reshapes the gums so teeth are framed in their natural proportions. For teeth that appear short, crown lengthening — performed with or without laser — carefully reveals more of the natural tooth structure.

  • Gingival contouring — corrects an excessive gum display for a more balanced smile.
  • Aesthetic crown lengthening — removes excess gum, and a small amount of bone where necessary, to make short teeth appear symmetrical.
  • Functional crown lengthening — when a tooth has broken down below the gum line, this exposes enough sound structure for a crown to grip, sometimes saving a tooth that would otherwise need extraction.
  • Gum depigmentation — removes melanin-rich tissue to reveal healthy pink gums in a single session.
  • Frenectomy — releases tight muscle attachments that pull gums away from teeth, preventing further recession.
See how this fits into saving a difficult tooth
Why Grafting Matters Around Dental Implants

Soft tissue grafting around implants builds a thick protective barrier — a "bio-seal" of firm, keratinized tissue that blocks food debris and bacterial ingress along the implant surface. Thicker gum tissue also makes daily cleaning more effective, helping prevent the chronic inflammation (peri-implantitis) that can lead to implant failure. Every implant patient at Excellence Dental is assessed for this before and after placement.

Flap Surgery &
Advanced Bone Regeneration

For advanced periodontitis, standard cleaning is no longer sufficient. We use a multi-modal approach — combining laser therapy and micro-surgical debridement — to halt infection and save teeth that might otherwise be extracted.

Halting Active Disease

  • Pocket Reduction (Flap) Surgery — gains direct access to tooth roots to remove deep-seated bacteria and smoothen damaged bone that non-surgical treatment cannot reach.
  • Laser-Assisted Decontamination — targeted laser energy disinfects infected pockets alongside surgery, with less post-operative discomfort.
  • Guided Tissue Regeneration (GTR) — biocompatible membranes guide the body into regrowing lost bone and attachment fibres.

Hard Tissue (Bone) Grafting

When the jawbone has resorbed from tooth loss, trauma, or infection, it can no longer support a dental implant. We regenerate lost structure to create a high-density foundation.

  • Socket Preservation — immediate grafting after extraction to prevent bone collapse.
  • Ridge Augmentation — restores the jaw's natural contour for stable, aesthetic implants.
  • Sinus Lifts — specialised grafting for the upper jaw to ensure safe implant placement.

Why Deep Cleaning by a Specialist
Is Different

A routine cleaning removes deposits above the gum line. Deep cleaning by a specialist periodontist is a fundamentally different procedure — and a fundamentally different experience.

01

Specialist Training

Our periodontists hold MDS degrees in Periodontics — three years of postgraduate specialist training beyond a dental degree, focused entirely on gum health, bone biology, and implant dentistry. This is not a skill acquired in a weekend course.

Three years of specialist training. Every procedure.

02

Thorough, Not Rushed

Deep cleaning performed properly takes time and precision. Our periodontists use ultrasonic scalers and fine hand instruments to remove deposits from every surface of every root — methodically, quadrant by quadrant. Patients consistently describe the result as transformative.

Thorough by design. Gentle by nature.

03

Gentle Enough to Trust

Many patients come to us after difficult experiences with gum treatment elsewhere. Dr. Pragya and Dr. Ruthali Kamat are consistently praised for making the procedure comfortable — often for patients who were anxious or had avoided treatment for years. Local anaesthesia is used routinely.

Anxious patients are our most loyal patients.
